Fixes #584. Adds CODE_OF_CONDUCT.md verbatim (byte-faithful copy of the supplied text, UTF-8, no non-BMP chars) and a blockquote reference line after the README intro. No code, no deps, no doc-amendment needed (standalone policy file, not a behavior change). Review: byte-diff of CODE_OF_CONDUCT.md against the issue fenced block is empty; README diff is the two added lines only.
